Ear position alone is not enough. Combine posture, eyes, breathing, tail, movement, location, recent events, and whether the behavior is normal for that rabbit.
A sudden behavior change accompanied by appetite loss, fewer droppings, weakness, or unusual posture should be treated as a health concern.
